Facility Use Request

Use this form to request room(s) at The Bible Chapel. All fields with an * are REQUIRED.

PLEASE NOTE:

  • This form is NOT for Wedding Requests.
    Wedding Forms can be accessed via the following links:

  • Allow two weeks for processing of your request. All requests should be made AT LEAST two weeks in advance.
  • Due to the large volume of requests we receive, allow for at least three business days for a response to your request. If you are not contacted after three business days, please call the main office at 724-941-8990.
  • Due to the large volume of requests we receive, we are not accepting requests for any ongoing sports related events that are not part of our church ministries.
  • Checks are payable to The Bible Chapel; designate on memo line: “Facility Use,” and give or send to the attention of Lisa Brooks.

Set-up:

Medical coverage is available only for Bible Chapel ministry events and if attendance will be greater than 100 adults and/or 50 children.




Donations and Fees: Please refer to the Facility Use Guidelines.
